I see online there are train connections between Orvieto to Montepulciano and Montepulciano to Rome. However, the RS book does not mention these train connections. Am I missing something here? Are there bus connections that are better?

I am not sure if Steves lists every train. If trenitalia shows the train it is there. Most likely it is a Regionale train so just a matter, buy a ticket once you are there, validate, and get on the first Regionale train headed in the correct direction.

That train station is very small and not many trains. I don't know if the buses are timed to the trains.
Just returned from there last week. From Montepulciano to Rome we took the train. We had a driver take us from Montepulciano to Chiusi train station. It was about 20 min drive and Chiusi is on the Orvieto Rome line. There are also buses from Chiusi to Montepulciano but I didn't see the schedule to see if they are timed to trains
