We contacted our hotel in Lake Como for the name of a car service to take us from Milan to Bellagio. We are dealing direct with the car service. What is the custom for tipping the driver? Do we or don't we? If we do, what percentage?

You don't need to tip. If he helps with your bags you might give him a few euros but you don't need to tip anything.

I wouldn't tip anything unless the driver was extraordinary in some way, and even then only a few euros at most.
