I have been unsuccessful ordering tickets for The
Last Supper online or by phone (line is always busy).
Does anyone have any ideas on what I might try? We will be in Milan in early October.

It is sold out on the site but you can buy a tour from a 3rd party company and even they have limited supply

My daughter, like you, was unable to pre-book a ticket and did not want to buy a packaged tour that included the Last Supper, so she just showed up and on that particular day in September, they had tickets available later in the day. Whether is was just a stoke of luck, or reflects they sell some/a few walk up tickets is unknown.
That plan works only if you are OK with not getting in. If you MUST see it on this trip, then the package tour route is the choice.
When I was there, another member of our party had purchased tickets online before the trip began. When we got there they had no record of our reservation. I had a copy of the email confirmation and they got us in.
