thoughts on my itenerary for Italy

Regarding train, I'll prebook Nice to Milan, but the rest can be booked a few days in advanced without significant price difference? Also I heard there's a train to Cinq Terre, I'm not sure the name of the stop. I was thinking to stop there from Milan to Florence. Should I just settle into Florence and take a day trip?

There are three different prices for train tickets. They cheaper tickets are limited in quantity and once they sell out they are gone. You "may" be able to get the super economy tickets just a few days prior to travel but it's doubtful. Most likely you'll pay the base price for tickets if you wait so long to purchase them.

I would drop the trip to the CT, you really don't have time for it.

Add the pantheon to day 1 in Rome, it's a 10 minute walk from Piazza Navona.

Milan to Florence I would book in advance for the best price
You can buy train ticket from Florence to any of the towns in Cinque Terre
it will require a change in La Spezia.
This you can wait until you get there. The price will not increase.

There is a direct train Milan to Monterosso (Cinque Terre).
Maybe go there first and then to Florence. But you should probably stay the night in Monterosso (or whatever other town you choose along the Cinque Terre) since you will have a luggage. i don't remember seeing lockers to store that and you say you plan a hike.

I'm going to agree with dropping CT. Takes time to get there, and as the 3* reason to go there is ambiance and charm, rather than say a big clock, spending half the day getting there and half the day there never seems like a good option. If you want a day trip from Florence, there are many. Alternatively, there's a Duomo, a Baptistery, a Campanile the Bargello, San Miniato al Monte, the bridge, and more that you're not hitting in Firenze, that make me think there's easily a third day of sights worth seeing there.

I might be biased, but based on my previous whirlwind tour, Florence was my favorite place out of 5. It might've been the gelato, but it might've just been everything else and the gelato too.

Thank you for all the great feedback. I'll reconsider the Cinq Terre trip and spend an extra day in Florence. Any suggestions on what to do in Florence outside of museums and churches?
