You do NOT need a guide to walk to the top of Vesuvio. It is an easy walk up. You can easily do it in tennis shoes. There were even some Italian ladies walking up in high heels (not advised) You could do it in Chaco sandals, but your feet will get very dirty/dusty.
The bus drops you off 1,000 feet from the top. You will need to purchase a ticket to get past the "guard house" (for lack of a better term) to finish the hike to the top.
You walk by the rim, just on one side..take binocs if you want to see if you can spot Pompei from the top!
