Hi everyone...I can't find this answer. We will be in Milan the first Sunday of the month when the last supper is free. Is there any way to secure a time slot or do you just line up early in the morning? Thanks for any help!

Actually, you have to make a reservation online or by phone, just like for any other day. The number of people that go inside is always 25 - 30 on 15 minute slots. I would say it's even more difficult because it's the day people from Milan try to go, so I would recommend going online a few weeks before.
