The daytime fixed fee is 40E. Four is the max for the taxi but luggage could be problem with four.

There is a fixed rate of 40 euros to any hotel inside the city walls. Make sure to let the driver know you are aware of this fare. Some will try to cheat you. Also, don't take a ride from anyone who approaches you and offers a "cheap" ride. Get your cab from the cab rank. This fare is good for 4 people with their luggage. I believe it is up to 2 bags each. For each additional bag it is one euro.

My wife and I recently returned from Rome. There was only the two of us and we took the Leonardo Express to the Stazione Termini. The fare was about 11 to 12 Euros each. It may be cheaper if there are more in your group to spread it over a taxi fare. If you do take the Leonardo Express, beware when you purchase your tickets. I got into a line that looked like an official train ticket booth but saw a sign on down that said "Tickets for Leonardo Express". So, since there was no line there, I went. When selling us the tickets, he tried to switch us to a much more expensive bus that would take us to the city. He said there would be no wait for the bus, but that the train would not be arriving for another hour and a half. I stuck with the train tickets remembering things that I had read on here about the frequency of the trains. Within ten minutes we were on board and headed to Rome. The only glitch was that the ticket stamping machines were not working so we boarded with our tickets ready to show in the event we were asked....probably not acceptable to the train officials but we were tired. The point of that is that I don't know if we were sold valid tickets or not, they were never checked. For the trip back to the airport after our vacation, we rented a limo for 55 euros through the hotel. We wanted a guaranteed delivery for our early flight plus the trip from the train terminal at the airport to checkin can be quite a walk. Good luck.
Beware of the crooked Roman taxi drivers. Get a ride only from a genuine taxi stand or you will get ripped off. In haste, I didn't heed my own warnings last trip and it cost me.
There are 4 of us and our place is a little off the beaten track after arriving at termini, so I figure 40 euro for 4 would be even cheaper than the train anyway and we can get exactly to our spot.

If you decide to take the express train know that they come every half hour at 6 after. If the machine is not working to stamp your ticket you must right the date and time on your ticket or you could be fined. This is with any form of public transportation in Rome. It is cheaper to take the train but you will still have to get to your hotel once you get to the termini station. If you are staying in the area and can handle your own luggage you can simply walk to your hotel. If you are not staying in the termini area you can still walk to your hotel or take the metro, a bus or a cab to your hotel. The metro is limited on the areas it covers but you can get close to most of Rome on it.

Kevin, You may also want to consider a private driver. They are usually only 5-10 euros more than a cab and will help with your luggage and take you directly to your hotel. They will be waiting for you inside the terminal just outside of baggage claim. This is what we do since we don't want to bother with waiting in line for a cab and the worry about the cab driver being honest or not. The 2 companies I use are romecabs.com and airportconnection.it. They are both great and have never let me down.

I would have a driver arranged in advance. It may be a few euro more than a taxi but the price is set, no worries about the sneaky taxi driver.
Once returning from Sicily to Rome we decided to take a taxi. The taxi line stretched forever and we had to wait 30 minutes in the blazing hot August sun for a taxi. We were kicking ourselves for not arranging a car in advance!
With the 40 euro flat fee to the city, does anyone add tip to that usually, if so, what is the standard?

Sure, but don't know the "standard." I would add a couple Euro IF the driver was friendly, helpful, and made the trip more pleasant.
I would give a few euros to the driver if he helps load and unload your luggage.

We were a party of four and used RomeCabs www.stefanorometours.com. We were picked up in a Mercedes Benz van and had plenty of room for all our luggage. Stephano is a true professional. Paid 55 euros from Airport to the Vatican Neighborhood (Prati).
Romecabs looks nice.... was the 55 euro including tip?
The Roman taxi drivers put up the fixed rate to/from fiumicino airport to 45 Euros,this is in a small car which might be going very fast.
A safe bet is to have your hotel arrange a ride to pick you up at the airport. It's still 40 Euro (up to 4 people) and they will meet you, holding a sign, outside of the baggage claim. Be sure to let them know your flight itinerary and how many people (and their bags) will need a ride.
I too had problems with CROOKED taxi drivers my last trip in May, and I knew what to look out for. Only grab a taxi from a taxi stand and not one that is sitting at the curb conveniently waiting for you. Some will also try to perform a "quick change" with your money, so be sure to say the denomination of bill you are handing them as you do so.
