Taxi Fare Flat Rate in Rome from FCO--even at midnight?

I have read that the taxi fare from Rome Fiumicino Airport to the Termini Train Station is a flat 48 Euros for up to 4 people, no more than 1 bag each. Then I read that it costs extra late at night. I will arrive so late, that after customs I expect to maybe barely make it to the Leonardo Express Train or the bus, about 11 to 11:30pm. Is there an extra charge for the late taxi?

I would use Romecabs.com. I've used them twice and they were extremely reliable. The cost is about the same as a regular taxi but they're more reliable and don't charge for helping you with luggage (as we had one cab driver do in Rome this past September when transferring from the train station to our hotel). We arrived into Rome 5 or so hours late on one of our trips and I had booked ahead of time with Romecabs and was worried that our driver would not know that our flight was late or would not be there waiting but sure enough when we exited the baggage claim area, our driver was standing there holding a sign with our name on it. He had indeed monitored our flight arrival time and was there waiting for us! The drivers are always extremely helpful and friendly and they use comfortable, clean and air conditioned cars. In September of last year, we used Romecabs once again for a very early morning transfer from our hotel to FCO. When I made the reservation on-line I had been told that there would be an extra fee since it was so early in the morning but when we paid our driver, he said not to worry about the extra charge which was very nice of him. And by the way, as early as it was, our driver was still 15 minutes earlier waiting in the lobby of our hotel and helped us with our luggage. They are highly rated on the internet and for good reason. Have a great trip!

The Rome to airport fare of €48 is all inclusive and is valid for up to 4 passengers.
There are no additional fees for baggage or night travel.

I would recommend Roma shuttle (romashuttle.com) as I used them two years ago and it was wonderful. It was only for the trip from my hotel at 7 in the morning to the airport and I had to go to terminal 5 for the return to the US. I was dropped off right in front of the terminal which made it extremely easy. I am returning this year and have booked my pickup at the airport (45 euro no extra charges) and return to the airport (40 euro no extra charges for bags etc). They are delightful to deal with (Alfredo is the general manager), and when you contact them they return a confirmation within hours. My trip isn't till July, but I have oth ways booked, andcan now relax about this part of the trip. They were recommended to me by Ron, from RoninRome. Their e-mail is romashuttle @yahoo.it, I don't think that there is a limit to how many passangers either (with-in reason, say 4). Hope that you have a wonderful trip. This trip for me in July will be #6 since I retired 7 years ago, and I only wish that I had been able to afford to travel when I was younger (over 70 now) but still loving to go to Italy!
