Suggestions needed for best itinerary management

The points on the itinerary are Milan (or Genoa), Cinque Terra, Florence and Rome. Ending (or starting) in Rome is the desired route, as I plan to visit the hometowns of my grandparents about 2 hrs from Rome (toward Pescara). I have been to Rome before so don't plan on being there more than 2 days, but everywhere else would be a new adventure to me. Traveling within Italy by train and going in September.

Does anyone have suggestions for the best "flow" given the planned stops to avoid wasting time?

What is the hometown, and do you need car to get there?
Into Rome and out of Milan (or Genoa) or vice versa would work fine.

My grandparents came from Pratola Peligna in L'Aquila, which I understand from looking at train schedules is about 2 hrs from Rome. I could drive, altho having been to Rome once before, I'm not too sure about making it out of Rome alive by car LOL!

The tiny villages of the Cinque Terre [that's with an E] get 2.5 million visitors per year, so during the season expect suffocating crowds.
