Struggling with dPLF Form for Entry into Italy from Croatia

Hello,
In a couple of days, my husband and I will be traveling to Croatia and staying there for 13 days then flying over to Rome from Split. I'm struggling with filling out the dPLF form as I'm not sure if I should just fill it out starting from Croatia or need to start it from the US. Since we're just shy of 14 days in Croatia, I assume I need to complete it starting from the US and completed mine that way. Thought I would ask if anyone else has traveled into Italy from another country instead of straight there from the US and how you completed the form.

I don't have experience with a stay in another country abroad but I would assume, unless you hear otherwise, to fill out the form as if you were leaving from the US. I don't know if you will be questioned if arriving from another EU country but you will be prepared in case. Good luck!
