Strenuous activity level

We are looking into the Heart of Italy RS tour and I am concerned about the amount of walking. I've had an ankle and knee replacement. Are there places to stop or sit down along the way on tours. What if I can't make a tour, one day?

You can skip anything you’re not up to but it’s harder on the bus transit days to do that. Often there are not places to sit.

Rome has a lot of stairs so I would start bumping up your activity there if you can. If you get tired on the Rome day you can hop into a taxi and head back to the hotel.

In Volterra the bus takes you up (or did when I took this one in 2013) to town but you have to walk thru town to get to the hotel as the streets are too narrow for the coach.

In Monterosso al Mare you’ll get off the coach in Levanto, take the train to Monterosso where you’ll walk to your hotel. The full day in CT is free so you can have no activity this day if you choose.

Florence is flatter but there are some stairs in the Uffizi.

Can't specifically address the Uffizi, but a number of museums have elevators for people with mobility issues (ok, some of them may be freight elevators, but if they offer it I will take it.) Sometimes you need to ask, sometimes just looking frail and/or elderly will get you an unsolicited offer.
