Is it generally necessary to buy advanced tickets to music events at St. Paul’s Within the Walls in Rome, or are tickets usually available at the door? Thanks?

I would suggest booking tickets in advance to secure seating.
Depending on the day and month, it may vary as far as booking in advance or securing same day ticketing.
I am not sure how seating is set up now, but when I attended a concert in the church, it was an absolute lovely experience.

I've attended a number of concerts at St. Paul's Within the Walls over the past 5 years and I normally go with a group of 15+ students. With the group, I usually drop by and get tickets a day or two in advance. If it were only me, I'd feel comfortable buying them the same day as the concert, but not in the time immediately before the concert begins (i.e. not walk up).
