We are touring Sicily in March, doing Positano before. We have 4-5 days on the back end before flying home from Rome. Since the tour ends in Catania, we are considering flying to Naples and renting a car or seeing parts of Sicily we might have missed. Any suggestions would be welcome
Fly to Rome and enjoy those 4 days in Rome before returning.
If you’ve seen Rome already then you could consider staying in Sicily longer and visits parts in Eastern Sicily not covered by the tour.
I wouldn’t fly to Naples and rent a car. To go where? Besides if you plan to visit the Amalfi coast before Sicily, you should see Naples then.
It sounds like you have purchased your flights to/from Rome?
If not, there is probably a much better way to arrange your trip by flying out of a different airport than your arrival airport.

Any specific towns/villages in Sicily that the tour doesn’t cover?
I presume you are referring to the RS Best of Sicily tour. Never been on that tour but I don’t think it covers Noto, Ragusa, Modica, Marzamemi, Palazzolo Acreide, Ferla, Caltagirone.
They are all in Southeastern Sicily, not far from Siracusa o Catania. You could rent a car and visit some of them.
