We have stayed twice at the B&B La Magnolia, a family owned B&B just down the street from the cathedral. Nice rooms, clean and comfortable. Breakfast is provided in the bar next door, also owned by a member of the same family. Here is a link:

Another vote for Hotel Duomo. Wonderful family-run establishment that is convenient to anything you wish to do in Orvieto.
Another vote for Magnolia B&B. We spent one night there last month. Good location, good breakfast.
We love Magnolia B and B. Ask for a lower floor, however, if stairs are an issue for you.
Based on excellent reviews, value and location we reserved Hotel Virgilio in Oct.
I was very happy with Hotel Duomo. I ended up taking a cheaper room in their annex. It meant walking about 5 minutes to the main hotel for breakfast but was worth the savings for me. For a splurge, get a room with a balcony.
If you have a car, I'd highly recommend Locanda Rosati (agroturismo right outside the city). We stayed there a couple years ago and had a wonderful time. https://www.locandarosati.it/en/
