I guess it depends on how long you wish to spend in a car, but you could head north, see Pietrasanta and a loop of the coast at Tellaro, perhaps see the marble quarries at Carrara if the day is particularly long.
You could go to the bigger towns of Lucca and Pistoia, maybe Bagni di Lucca.
South to Massa Marittima, see Abbey of San Galgano.
South to Volterra and San G--the options are really limitless. I am sure there are organized trips there already from the boat so maybe you are looking for less crowded?

when is this cruise and when are you in livorno and how many hours?

great site for all cruise ships in ports. find your ship, port, date. let's you know how many on cruise ship and in port that day. which adds thousands more in the ports, crowds, restaurants/shops full, trains over packed, many wanting to go to pisa/florence in a sjort time since travel takes up alot. stay close to port. is this a docked or tendered port, how far to walk or shuttle to main entrance?
like valadelphia idea of the tour to pietrasanta and doing a loop. have you hired the private driver yet?
are you registered on cruisecritic.com? the few cruises we've been on i would post a small private tour (6-8 people) and if anyone interested to share cost. it was first come first serve. we had people say yes really fast. found a taxi/tour driver, what we wanted or let him put input for what he thinks was best, very adament about tour getting us back to port before departing. let others know and if they wanted to add something for themselves and all worked perfectly.

We have been to Florence and Pisa several times so we were hoping to do something different; We have also had the same
private driver for several trips. The ship has comp tours of Pisa and Florence but I booked late so they are saying the tours are full
wah wah My husband is not walking great anymore so I was looking for somewhere , some cute village to check out or maybe a couple for the day.
