Self-Declaration for Entry into Italy From Abroad

Is this form still required for entry to Italy? I've heard that the digital Passenger Locator Form replaces the declaration.

I've already completed the locator. Is the self-declaration required too?

no, the self-declaration is no longer required. Just the locator form and your CDC vaccination card. I don't have an official citation for this, but my parents and sister were just here in October from the U.S. and those were the rules they had to follow.

Hi Nelly.

KInd of a related question. A couple years ago when I started planning our Italy trip there was a requirement to notify the health department or Municipal office in each Commune that we would be in the area. I don't know where I saw that at the time (...it was on one Italian government site or another). I haven't seen it mentioned lately on any of the government sites I've been on. Do you know if that is still the case?

Or is that the self declaration Hank is asking about?

I believe that it is still a requirement to notify the local health authority in the Italian region that you plan to arrive in if you are UNvacainated as you will need to be quarantined for Five days and tested again at the end of those five days

I am in Italy at the moment and have not been asked for the self declaration form nor the dPLF. I have also read that the self declaration form was replaced by the dPLF specific to Italy.
Travel has been smooth, vaccination proof is asked on high speed trains and intercity trains not the regional trains.
Good luck on your travel planning! Of course, things may change - meaning document requirements.

UPDATE: We are home from our Transatlantic cruise. The only person interested in seeing our digital Passenger Locator Form (dPLF) was the American Airlines agent that checked us in at the Las Vegas Airport. No one in Rome FCO asked to the dPLF nor our CDC Vaccination Certificates. An automated system did check our temperature before we could enter passport control or baggage claim. I don’t exactly recall which after traveling 18 hours.

Bars and restaurants in Rome did ask for our CDC Vaccination Certificates which were accepted.

Also, we couldn’t get into the Colosseum, Forum nor Pantheon because we had to have reservations with proof of vaccination and QR code. Having been to these sights before, it didn’t occur to me that reservations would be required. Live and learn.

Given what’s happening with the latest round of restrictions, your experience could be different. COVID-19 marches on.
