We are hoping to visit the church and the crypts below. Do we have to book a tour or may we go into church then the crypts on our own? Headphones?

It has been 5 years since i was there, but at that time they had regular tours in several languages. You enter the church but you have to pay and be led by a tour guide. The tunnels are winding and the guides help keep track of you. I'm 6' and some random spots are 5' 10" so watch your head or the tour can be a headache.

You don't need to book a tour. Just show up and pay the entry fee. They redesigned the crypts so now you go through a museum before getting to the crypts. There is a lot of really good information in the museum. I don't see any need for audio guides as the museum signage is very good and the crypts are self explanatory.
