Rome to the airport in the middle of the night

We have a 6:30 a.m. flight to the US from Rome, which will require us to arrive at the airport at around 4 in the morning.

What transportation options might we have to travel to the airport after midnight? We will be staying at a hotel next to the train station.

Is it direct non-stop from Rome to the US? Or do you change plans somewhere in Europe? If the latter, you don't need to be at the airport that far in advance because that's a domestic flight.

There are no flights from FCO to the United States that early in the morning.
You are certainly going to some other EU hub airport first (AMS? FRA? MUC? BRU? CDG?).
Those are intra EU flights. The requirement is for you to have completed the check in procedures at least 45 min. prior to the departure, therefore getting to the airport 75 to 90 min. prior to departure is sufficient.
For a 6:30 flight you should try to arrive around 5:00-5:15.
At that time in the morning your best option is a taxi from your hotel. The taxi ride is approximately 30min from the city center (there is zero traffic that early in the morning). The cost is approx. €50.
Your hotel can arrange the night before a taxi to be in front at your hotel early the next morning. If you leave your hotel by 4:30 you'll be at the airport Terminal 3 (your terminal) by 5:00.

At that time, it will not be a direct flight to the US but a connecting flight in Europe and you DO NOT use the international terminal (Terminal 5) You could also use a shared shuttle. We have used a shuttle twice and it worked well. Although it will only be 10 to 12 euro cheaper, I would still do the taxi. Have the hotel call the taxi and be certain that you get into the taxi called by the hotel.

What they said. The hotel will probably give the taxi your name so you'll know it's yours. There may be an additional night supplement charge at that hour, that may or may not show on the meter, but that is standard practice.

We used Romecabs.com twice for transfers to and from FCO and would use them again. They're very highly rated and reliable. We had a very early flight out of FCO (to CDG/Paris) and used Romecabs this past September. They are always on time, the driver is always friendly and helpful and the cars are always comfortable, clean and air conditioned (I couldn't say the same for some of the taxi's we used while in Rome). The cost of a transfer using Romecabs is almost exactly the same as a taxi. We were told there would be a small surcharge for such an early morning transfer but in the end, the driver told me not to worry about the surcharge which was very nice. The Romecabs driver insisted on helping us with our bags at no additional cost whereas the taxi driver that drove us from the train station to our hotel added an extra €2 per bag. You can book your transfer ahead of time via their web-site and you don't have to pay until the day of service. Have a great trip.
