There are several companies that offer guided tours in Rome. ThroughEternity says its recommended by Rick Steves, Viator also has several offerings. Anyone had trouble with either one? Should you avoid the Vatican on certain days?
I did the early morning tour with Through Eternity, it was very good.
Victor books tours with other companies, they do not run their own.
I used Walks of Italy a couple of years back, they were fantastic.
We used Angel Tours and even my then 10 year old daughter thought this was the highlight of the trip to Rome! We did the Vatican tour and their free Pantheon tour, both were excellent and well priced.
We did a Context tour "Antica Roma" and the "Pristine Sistine" tour with Walks of Italy. We liked them both a lot. I would not do the Vatican without a tour and even then you should expect it to be crowded unless you are going very off season.
I usually just book through the official websites. Those tours are always very good and very inexpensive.
I would avoid the Vatican museums on Saturday, Monday and free Sunday.

We will be booking with Walks of Italy for the Colosseum and Vatican. We chose them based on the great reviews, good selection of what we are interested in seeing and price (you can get a 10% off discount with a Rick Steve code). We will also be using them in Florence. I looked into Angel Tours and Dark Rome also, as they all seem to have the same great reviews...Walks of Italy won for us, plus the only one with the discount.
There are several tour companies in Rome which offer discounts - see Rick's 2015 Italy, pages 844 - 845. I booked Through Eternity's early Vatican tour and received a 10% discount.
We are doing the Early entry Vatican also...both look like great tours.
Last year we took four tours with Walks Of Italy. One in Venice, Florence, Rome and the Vatican. Only one tour ( Colosseum) had 12 people. The others, less than 8. The guides were very informative. We thought the tours were excellent and worth the money.
