Have a look at Rick's At a Glance picks. Without knowing what you've seen before, or what your interests are, it's hard to suggest anything. Remember that you may not disembark that early, and they usually want you back on board an hour before sail away. How do you plan to get back and forth from the city?
Have you been to Tivoli with all of it’s fountains? It is a beautiful place. We took a day trip there from Rome. Rent a car or hire a driver for the day.
Ostia Antica?
If you are willing to rent a car, then maybe explore Umbria, the best options would be Orvieto, Civita de Bagnoregio, or both. There are probably other things along the way.

...what's near civetecchia rome to see..... If you mean - Civitavecchia - not a lot. Typical industrial port city. Just outside a little to the north, northeast is Tarquinia. It has a lot of Etruscan tombs and is older than Rome. More than worth a visit. Would have to rent a car as public transit is limited. Maybe a tour from Civit...... Goggle it !!
PS The above suggestions, especially Tivoli on the other side of Rome, would involve a lot of travel. You may want to stick closer to Civit.....
Your ship no doubt offers tours to places other than Rome from the port. Have you considered one of those?
Frank was correct when he said "...Civitavecchia - not a lot." Since that's the case, if you plan to go very far from the port, consider taking one of the ship's excursions. I know, I know, they do cost more than what one can do on one's own. But, if you are on a ship tour and there is a delay returning, the ship will wait for you. That's not the case if you are off on your own. Traffic (bus, train, and car) can be a real problem that can cause delays getting back to the ship. Naples is particularly bad for traffic jams. We seen people literally running along the pier as the ship departed. Don't mean to rain on your parade, just offering food for thought.

That's pretty much the problem here. There's also the question of who "we" are? As you'd noted in a previous post, you were looking for a 4-bedroom accommodation in Palermo so it looks like you're traveling with a fair amount of people? Children? Elders with mobility constraints? The larger/more diverse the group, the slower it can be to get around.
Were you planning on doing this day trip independently or with a tour? Again, the more of you there are, the pricier tour options can be although those with a flat group rates versus per-person fees can sometimes be advantageous.
Even though you've been to Rome before, you likely didn't cover ALL of its corners so it would help to know what your group HAS seen and the types of things they're interested in doing?
You need to look at the tours offered by the cruise line due to your time in port for that day. Cittavecchia is a port town and is about +40 miles from Rome. If you are going to try and do something independently the driver needs to meet you at the ship. If you are going to to do on your own and say by train to Rome, there is a lot of transit involved including a shuttle to the front of the port, a bus or taxi to the train station and at least a one hour train ride one way to Rome Termini.
Maybe Rick's Mediterranean Cruise Ports guidebook offers some non-Rome suggestions for the stop in Civitavecchia. If you're lucky enough to still have a bookstore nearby, you could stop by and thumb through a copy before deciding whether to buy it.
