My husband and I have been to Rome a few times. We stop in Rome (Civitavecchia) for 12 hours on our cruise. He has severe knee issues and it's going to be difficult to do a lot of walking, ie, no castle or Pompeii tours this time. Any recommendations for a day in Rome? We will have to take a train from the port (new experience for us). I'm just concerned with the walking from the ship to train, then from the train to whatever we do for the day. Also, how easy is it to get to/from Rome and back to the ship?

I believe there’s hop on, hop off busses you can take. Not sure where they go or what you see.
We usually take a cab to get us to and from a central location so that we can save our limited walking abilities for the interesting bits!
According to Mr Google, there is a cab stand at the terminal, plus links to Welcome Pickups (I've used happily before) and others.
Cost seems to be about $125 to $150 one way.
Depending on how much you want to see, the Colosseum, Forum and Palatine Hill are all close together, plus restaurants.
You don't give the month of the year or the actual budget requirements. For example, it sounds like you should take a taxi from the cruise terminal to the Civitavecchia station. But if there are a dozen ships unloading at 8 AM, he may not get a seat on the frequent local trains. Vast numbers of people use 150 Euro (??) car services to get to and from Rome for cruises. The ship's bus excursion, similar cost level, would eliminate a vast amount of walking, but might hit highlights you have already seen. But at least then the ship will wait for a late return.
Note that the train may have multiple stops in Rome, which could save steps for some less obvious sights.
I don't know about the transport costs, but a day in Viterbo might have less walking.
Have you considered not going into Rome? This is a location you have visited previously so I assume you saw your top items already. The port is not convenient to the city and significant walking will be required. Why not give your husband’s knee a day of rest?
