Would it be best to rent a car at the airport and take a chance on being hopelessly lost in the city? Or is there convenient transport to a nearby town where navigation to Tuscany might be simpler?

The Rome airport is outside Rome. IT is easy to hop on the Autostrada and head north.

There are extra fees is you rent a car from the airport. I don't know where you are headed to in Tuscany but you may want to think about taking the train to Chiusi and picking up the car there.
