I need to set up transfers from FCO airport to San Giorgio Hotel in Civitavecchia and then pier to Opera Roma hotel in Rome, then Opera Roma hotel to Hilton Airport hotel. Any experience with reasonable and reputable companies? Need info as soon as possible. I am going on Med Cruise and need to make these transfer arrangements.

You can take the Leonardo Express to Termini and then a Eurostar or Inter City Express (ICE) to Civitavecchia for o/a $30 each way per person (to the port that is from Rome Termini). Take an OFFICAL cab to your hotel in Civitavecchia. Upon return take the train from the port back to Rome Termini, Leonardo Express to the airport and then get the hotel shuttle to the Hilton (or a cab). Check out www.raileurope.com I ordered my reserved seating tickets on line and had them in a week. If you're traveling heavy which is curse of 1st time cruisers or if some physical limitation restricts you then the cab line mentioned above is safe solid option. O/A $150 each way I believe.
I would like to stay away from rail as we are in our late 60's and do not want to be hauling luggage thru rail stations, up and down train steps etc. Thanks for the great advise anyway. Wish we had younger legs and arms.
Unless you are locked into all your hotels, you can make this trip simpler and less stressful with fewer hotels. We have made that run four times. There is really no need to stay in Civit... assuming your ship has the normal 3 or 4 pm deadline for boarding. You can take the Leonard Express (14E/each into Rome) or taxi (40E fixed) and stay in a hotel near Termini and enjoy the evening in Rome - there is nothing in Civit..;. Train to Civit... is a commuter run with three to four trains an hour taking between 45 mins to 1-15 depending on train at a price rang to 6 to 12E. The Civit train station is less than a half mile to the entrance to the port - shuttle, cab, or walk. We walk. It is flat and along the beach. Leave Rome late morning and arrive in plenty of time to checkin. The return to Rome can be the same. If doing that buy RT tickets in Rome is save the hassle of buying tickets in Civit... Depending on time of departure, a shuttle from your Rome hotel is convenient and reasonable. Unless you have a 5 or 6 AM departure from Rome there is no advantage to the Hilton. ................PS Just read your response that you posted while I was typing. We are in our late 60s and it is not problem with luggage but we have been doing it for the past 20 years and we keep our luggage for cruises pretty light.
Then you only alternative is hired drivers/cars - try Rome Cabs. About 150E to Rome from Civit and around a 100E to Civit from the airport - both on the same side of Rome so it is a much shorter trip.

I received a reply from Rome Cabs and they answered all my questions along with giving me rates. They don't ask for a credit card but work on honor system they said. Then I would either pay cash EURO when I receive the service or they said something about paying thru Paypal which I could do also. Thanks to all of you for your imput. I will report back on Rome Cabs when I return from my trip. There are sooo many companies online I did not know which were reliable and which were not. Again thanks to all of you.
