I would like to rent a car in Florence to explore the Tuscany region including Montelcino and Montepulciano with 2 nights in that area. Should I return the car to Florence or drive it down to Rome and return it there?
Drive to Rome. If you don’t want to do an inner city drop, drop it off at FCO, the airport. Taxis have a flat rate into Rome or take the Leonardo Express train.
I too would drive to Rome. Drop it off at Tiburtina Station, about 7-10 miles east of the airport, same exit. Then Taxi into the city, 8-10 minutes about €15.
Roma Tiburtina is easy to reach, but it’s not the same exit as the airport. You must follow directions to NAPOLI, then ROMA CENTRO/TANGENZIALE EST/A24.
If you go all around staying on the A1/E35 (to avoid the traffic on the GRA Beltway (A90), which I recommend during the afternoon commute, you will enter the city from the east. Follow directions to ROMA CENTRO, ROMA EST, A24.
You will stay on the A24 till the end.
Roma Centro (Rome city center) is often signaled by ROMA followed by a target symbol (Like the Target store logo). The target symbol means city center in Euro road sign speak.
Thanks for the responses it looks like now the rental car drop off is in Naples. Has anyone ever experienced that before?
We would need much more information about the itinerary, but Naples airport is a fine place to drop a car. Naples city, nuh-uh.
Are you going to stay in Naples? If you are actually staying in Sorrento, as many do around here, there are rental offices in Sorrento.
Regarding the wisdom of returning the car downtown Naples...oh boy!

Is it sketchy and dangerous? Don’t go there after dark type scenarios?

Naples is wonderful--I just would not drive a car in the city (traffic anarchy; ztls; yes crime, but no worse than any big city, etc.). Drop it at the airport and enjoy. We still don't know what your plan is or why you have the car there--it is not recommended to drive along the Amalfi coast either, unless you like sitting in traffic and paying exorbitant parking fees.
If you can deal with total road anarchy you can drive in Naples.

Neapolitans are the best drivers in the world, unfortunately they are not used to less gifted drivers in their streets. What some call anarchy, it's a perfect dance from their point of view.
Unfortunately we can't hear that music, so we'd better wait near the wall and watch the cool guys dancing.

Funny! People from war zones (or big cities, same difference) deal with things going wrong on a daily basis.
One more question. Are you entirely skipping Rome? If you are planning visiting Rome for a few days. Better to drop car off before Rome and not deal with driving and parking in Rome. You can always rent another car when leaving Rome and drop off car in Naples.

We are dropping in Naples city office via Giordano Bruno then taking the water ferry the next day to Sorrento.

Along with the somewhat chaotic drivers, (they know what they're doing) motor scooters who have the right of way, you have ZTLs, one way streets and numerous bus lanes which you could get tickets up to a year after your return home. Don't even think about using a GPS.
The taxi ride will only cost 18-25 euros. Take it.
If you are going to Sorrento you might as well drop the car in Sorrento, much easier than Naples. You can experience the boat once you are settled in Sorrento and visit Capri.
