we are renting a car when we leave Rome and I'd like to know if it's better to pick it up at the Termini station or one of the airports given the complexities of driving in the city. Thank you for your input!

Where are you going with the car after you pick it up?
As a general rule, picking the car up,at the airport costs more because of various airport fees. Also, if you are already in the city, it makes little sense to spend 50€ on a taxi ride 20 miles out of town to the Fiumicino airport when you can pick up the same car from the same company (Europcar, Hertz, Avis-Budget, or whatever) at a city office for less than 15€ in taxi ride and at a lower rental rate as well.
Tell me which rental company you are using, where you are staying in Rome, where you are going after pick up, and I will give you a choice of sensible locations. Just know that all major rental companies have about a dozen offices in Rome, including the two airports.
I have this same question and will be staying in Trastavere and renting from either Avis, Eurocar, or Budget.
You need to know where you are staying, which company, and where you are going to make a decision. Or you can rent at the Termini station, which is pretty central, and go from there. But if let’s say you are stayin at Prati and going to Tuscany, then there are offices that would make more sense than going down to Termini. If you are staying at Trastevere, same story. We need to see where you are headed. If you headed to Naples I’ll suggest a different agency than if you are headed to Florence.
For years we picked up our car through Autoeurope from Europecar at Stazione Termini without incident. In fact, we rent from Autoeurope every year and prepay when we are in Italy. This past September we retrieved our car from Termini and the Europecar/Autoeurope employee gave us an upgrade and charged us for it. We were in a hurry and they were slow, so we missed the charge. The next day I received a text from my credit card company presenting the up charge. We immediately contacted customer service at Autoeurope and advised we recontact them when we returned home, which we did. Autoeurope refunded the entire up charge amount and we will use Autoeurope again. Now, this was the first time this has ever happened in 22 trips to Italy since 1980.
I posted this event on this forum and one other and was promptly told repeatedly how stupid I was for allowing myself to be a victim. I suppose those critics are always more alert than I, but, at any rate, Autoeurope is terrific and their customer service is most effective.
So, in my long winded tale, using Autoeurope is a good choice and they have other locations in the city. Driving in Roma requires patience, but it is not so impossible.
