With 3 or 4 nights to spend heading south from Siena to Civita/Orvieto, are local buses reliable or would you recommend renting a car? Thank you.

Buses are reliable, but service to small towns is infrequent, especially on Sunday. To maximize your visiting time I would definitely recommend renting a car for that area.
Use a consolidator such as:
www.autoeurope.com
www.kemwel.com
(They are sister companies and work similarly but check prices with both).
They work with major rental companies in Europe, such as Europcar, Hertz, Avis, etc.
Your pick up location is Siena. I don't know where you are going afterwards so I can't suggest a drop off location. Hertz is the only company with an office in Orvieto.
