To get the Codice Fiscale, go to this web page. it will provide you with the number as if you have received an official one from the Italian government but it does not mean you are actually registered. In the box for town of birth, enter Stati Uniti for USA.

I don’t remember if I entered a “local” address or not. If I have to enter an address in Italy, I generally use the address of the first hotel on my trip.

Yes, it is easy to do if you correctly insert the country name. If you are seniors, there is a way to get a free senior discount card (carta freccia) but you can’t do this online as a non resident. You can fill out a form, scan it, email it in, and wait for processing.

I did not have any problems registering on the Trenitalia site using a US address. Note that in the pulldown you need to look for "Stati Uniti" for the country, not "United States" I was able to then use the app to purchase tickets and display them to the conductor as needed.
