Can anyone recommend a private car service to bring our family from MXP to our hotel in central Milan near the Duomo? We are very hesitant to take the express train or a shuttle after our long flight. Thanks!

Actually, taking the Malpensa Express from MXP to Milano Cadorna is not a huge effort. Many here have done that without any problems. The trip is about 35 minutes. Once you arrive at Cadorna, take a Taxi to your hotel (it will be a short trip).
If you decide to take the train, post another note here and the group here can provide more specific instructions on the steps to follow.

Malpensa is far from Milan. The train to the Cadorna station is easy, comfortable and much faster than the alternatives.
The cost is also much lower unless you have a large family. The taxi rate is €90, while a private car transfer might be a bit more expensive. The train I think is only €10 per person. From the Cadorna station to the Duomo is a short walk, but even by taxi it wouldn't cost more than €15.

Note that it confirms what Roberto said - there is an official rate of €90 from the airport to the city center. So, there's no need to pre-book a private car service - just get in a taxi (in the official line - not someone who approaches you in the airport) and confirm the rate. You would only need to pre-book a taxi in the opposite direction, if you were staying somewhere not near a cab stand, or for an early departure.
