Is there train connection to Milan from Monterosso (or close to)?
Or how else to get to airport? How much is car rental option just to get to airport

Yes, there are definitely train connections from Monterosso al Mare & the Cinque Terre to Milan! The easiest and most direct solution for transport to the airport would probably be the Airport Bus from Milano Centrale station to Milano / MXP airport. There's also an airport train, which is accessed from another station in Milan (can't remember the name of the station at the moment).

If you have to google in connection with your trip to the Cinque Terre, it will be important for you to get the spelling right or at least close.
