I cannot find the name of a restaurant we visited years ago which was recommended (probably Italy 2000). It's a family-owned restaurant and they only serve one menu at a prix fixe. Near the Pantheon, I believe, and tucked back in a little side street. Any guesses? Thx!

Sounds like Trattoria der Pallaro, which is on a side street near Campo dei Fiori. They serve one menu each evening for a fixed price.
I agree with Frank. Trattoria der Pallaro is at 15 Largo del Pallaro, their motto is "you will eat what we want to feed you". There is no choice.. it is a 5 course meal, served family style. I was there a couple of years ago and would definitely go back. They are closed on Mondays and are open for lunch and dinner Tuesday - Sunday.
