Any suggestions for a private transfer service from Civitavecchia to Roma Termini?
Also…when using a private transfer is it customary to tip the driver? If so how much?

Google a company named "Rome Cabs". It is a private car service and not a taxi company despite the name. We've used them a number of times in Rome between the city and the airport and have always been happy with their service. We always email them with our needs and hear back from them within a day. If they can not accomodate your needs, I would feel okay dealing with a company they suggest. As to the tip, that usually (for us) depends on the amount of help with luggage and etc. But it's just a nominal amount, and nothing like what is expected in the states.

Help with luggage or not, it's customary not to tip anyone in Italy. A driver that owns the car and the license can't help someone and then expect his customers to pay more.
Unfortunately some waiters and drivers may tell you that "service is not included" the moment they realize you are from US. Of course they wouldn't dare tell Italian customers the same because Italians would tell them to get lost.
The choice if being subject to a double standard or not is yours.
