Is there a particular tour company that uses private vehicle and guide from Civitavicchi to Rome? Also Naples to Pompeii? We will be on a cruise ....total of 5 people. Thanks.

We and two other couples used www.romecabs.com from Rome to the ship in Civitavecchia. Excellent service. They do tours as well so could customize an itinerary. Highly recommend.
Is there something specific you want to see or do? You can easily take the train into Rome for less than 10 euros each. A private transfer will cost hundreds of euros. If you want a city guide I suggest taking the train into Rome and meeting the guide at a specific spot. They can take you on a tour of the city and get you back to the termini station in time to take the train back to the port.

Rick's Mediterranean Cruise Ports book (which is on sale today!) recommends www.montitours.com (see pages 514 and 529) for those who want that sort of premium service into Rome, but it certainly is easy enough to take the train into Rome. He also provides several resources for Naples, and among that advice describes that you can negotiate a set price with the taxi drivers at the port, settling on exact destinations and number of hours they'll wait at each.
