Not to sure about barcelona, i just did a bus tour there, but for italy i was there with a few people and we organised a private tour with this company http://www.wheninrometours.com/ they are called when in rome tours and we had a guide called annemarie, she was great, we even ended up booking on to the pompeii tour which was brilliant and so lovely to see sorrento too.
While there are many excellent guides in Rome, I feel there are none better than RoninRoma.
He assisted my wife and me back in February extensively beyond simply being our guide for a day -- which was well worth it even without the additional help.
Use him if you can!

Yep..Ron in Rome. We spent a lot of time with him back in Oct. Couldn't do better! You tell him what you want and he'll take you there, navigate the city, or give you directions, and he knows tons of information. Plus..he knows where all the great restaurants are!!

If you want a terrific tour of Tuscany-Siena also-call Luca at www.hillsandroads.com. Highlight of our last trip to Tuscany was a day tour, visiting wineries, lunch in the hills-really seeing the backroads we'd never have discovered. Luca has a wealth of knowledge and is a great guy!
While it is not quite a Private guide, context travel offers pretty good tours and experiences. We are using them in Rome and Florence.

A group of five of us recently used Through Eternity Tours for seeing the Vatican. We had a guide named Marle and she was excellent. Very knowledge, polite and professional. We got our money's worth.
