If you're staying in a hotel or B&B or agriturisimo, check with them to see of they know of a driver.
Otherwise, it may be a bus to Florence and then a train to Lucca.

That's about a 2.5 hour trip each way so I would expect a driver to be a several hundred euros especially since you are wanting to visit Lucca then return to Montepulciano- that's a long day and the driver will expect to be paid for waiting around for you.
