I usually figure my hotel will recommend a place that has pleased clients in the past. Some of our Rome hotels have offered a free transfer to the airport or to Civitavecchia with a minimum stay of 3 or 4 nights. Because your boat probably doesn't leave until 4 PM (made up time ... ) the risk of a late arrival is small. But I certainly recommend your plan.
We tried the train once, and it was really cheap, but it stops rather far from the port, if you have any substantial luggage. The bus is infrequent and full (or was for us), and cabs expensive. Try to find out if you are going to a cruise terminal, or possibly as we were, directly to a smaller ship. We still had to take a port-shuttle bus (49-passenger) from the port gates to the actual pier.
Like toddw, we've also used Rome Cabs and have been very pleased. It will be expensive, but worth it if you have more than just carry on luggage. We've also taken the train, and while it was inexpensive, it was a bit of a hassle. If it fits your budget, spend the money for the car service and go in comfort.
