Hello All,
We will be dropping our rental at Naples Airport and heading south to Positano. We will be coming to Naples from Umbria. Goggle maps puts the trip at about 3 1/2 hours, but it will be on a Sunday, and so we are a little unsure about arrival time in Naples, dropping the car and all that is involved in that. Do we need to book a cab in advance? Has anyone done anything similar, or have suggestions?

You don't need a taxicab in advance, even for a Sunday.
You would need advance arrangement if you used a private transfer (sort of a limousine service), instead of a regular taxi. The Italian word for private transfer is "Noleggio Con Conducente" (car hire with driver) often abbreviated with the acronym N.C.C.
Given the distance between NAP and Positano the cost would probably be over €100.
A money saving alternative could be to return your rental car at Sorrento, which is much closer to Positano. Most major rental companies have an office in Sorrento. Then from Sorrento you could take a taxi or a bus to Positano. If you decide to return in Sorrento, you need to verify that the Sorrento office of the rental company you will be using is open on Sundays. Many rental offices outside airport locations close on Sundays afternoons after 12:30pm.
An other money saving alternative is to take a shuttle bus from the NAP airport to Sorrento, then a taxi from Sorrento to Positano. Curreri Viaggi (www.curreriviaggi.it) operates a very comfortable shuttle bus from NAP airport to the Sorrento train station for only €10 per person.
