We both have orthopedic issues and had a difficult time In Positano last time. I need a recommendation for a hotel in the lower level.

It's an expensive, luxury hotel, but have you considered La Sireneuse? They have an internal elevator, too.

As I mentioned previously, you probably can't find lodging in Positano this summer if that is your intention.

Phred, we don't know if they "can't" find lodgings without knowing their actual dates and preference for per-night tariff. I looked earlier today for someone else wanting rooms at end of May, and while availability is tight and expensive, it's not like there is NO availability. You do have to be prepared to pay a steep - sometimes VERY steep - price depending on location and amenities.
