PLF help, please

I've read through this forum, but I'm still not certain what to do. We have one domestic connection and then fly to Rome. From there, we will immediately take a train to Bologna where we are staying a week before moving to 2 other cities in Italy. My questions:

  1. Do I select "Direct" and only list the flight that goes from the US to Rome? (I think that's what I read here someplace)
  2. Is Rome my "final" destination on the form (since we land there) or is it Bologna since that is where we are staying?
    3.We were just going to get on whatever train was available first, so I don't have a train number. Will I have to add that number onto this form after we buy the ticket or are they only concerned with where we finally end up (Bologna hotel)?
  3. We will be moving to another city after a week in Bologna. Will I have to put that on the PLF as well? I read the entire 68 page direction sheet and I'm going crazy...starting to wish we were just staying home!

Don’t give up! It did take us 2 hours to finally figure it all out the other night, but that included the attestation form for Sicily that you don’t need.

I would agree with previous poster’s answers. You don’t mention the airline you’re flying, but one area where we made a mistake was remembering the in Europe it’s day/month/year when filling in dates. I think British Airways actually used the name of the month, so that made it easier. Spouse ended up with the wrong birthdate for Sicily on the first try.

The good news was we took screen shots and uploaded to British Airways today when prompted and it was approved quickly. We’re off tomorrow.

Thank you for the answers!! We are flying American Airlines (and I thought I was making it easy on myself by arranging flights that didn't connect within Europe!). Ah, remember the days when travel was easy...

Patty, we are flying to Rome then connecting flight to Sicily. I noted in your response that you completed the PLF and attestation for Sicily? Is the Attestation within the PLF or is a separate attestation required for Sicily? Thank you and all on the reply, I am finding completion steps are not as clear and I do not want any omissions.

Oh boy - this just made me rethink our PLF. Since we fly from SEA to AMS to FCO I just used the information from AMS to FCO. Correct or do I need to add the SEA to AMS information? And is our temporary address our hotel in Rome even if it's only 1 night?

My husband and I sat down together and filled the PLF out bit by bit today. After a couple of tries we were successful. I've been travelling to Europe since the late 70s and this is the most complicated trip to plan and take. It's a different world.

ChristyB - if I were more text savvy I could copy the link from this forum topic, but I’m not and we leave for the airport shortly - type in “Pre-registration form for Sicily” in the Search box at the top of this page. There is a link in the first reply that I used and if you scroll to the bottom it’s in English so it gives you access to the Sicily form in English. I’m not sure when you leave, so maybe it won’t be required then. We’re flying British Airways and it was in their required forms for Italy and is a separate form from the EuPLF.

Donna- we fly two separate bookings. The first leg is LAS-LHR-FCO. We just needed the EuPLF & vax proof for British Airways on that one.Our second booking is FCO to Palermo on ITA after an overnight stay. So, we just listed the Rome hotel as our destination on the BA flight paperwork. We have our Sicily form filled out for ITA. We figured it was easier to do it at home before we added jet lag to the equation. We put both flights in for BA and it was easy to do, but I’m not sure if it’s necessary.

That PLF a form is a pain n the neck. It took me quite a while to fill it out. And I had to do one for my husband as well. I listed our home airport RNO as the starting airport even though the next connection is DFW to FCO and final airport is PMO. That may be more info than is required but I don’t think I’ll be faulted for that. I also did the preregistration form for Sicily for each of us and hopefully it’s just a formality.
