What kind of charger for phone?

The one you use now--- may need an adapter plug ( like this https://store.ricksteves.com/shop/p/european-power-adapter )

Best way tip at cruise end?

I have not been on a cruise but my understanding is a daily tip fee is automatically added to your bill ( ~ $20-$30 per day)

Any advise on best place to convert to Euros?

General advice is at a bank ATM when you get to your first European destination.

You will not need an adapter on the cruise ship. The ship provide both 110 and 220v outlets. On the cruise ship you will need nothing special other than your regular charger that you use at home.

Cheapest and most convenient way to obtain local currency is at an ATM in the country with your DEBIT card. Do NOT use a credit card for currency. On all of our curises the tip, about $10 to 12 a day is just added to the final bill. You can increase it or decrease as you wish. The old days of putting money in an envelope and having to your room steward are long gone. You can still do that if you like but putting it on the final is more common.
