we have opted not to take a car for first part of our trip - Florence-Pisa-CT- but the next part of the tour (heading to Barolo/Alba) will definitely require a car. Does anyone have any tips about where to pick up a car if we are based in Manarola; I do not want to go backwards to La Spieza and if the nearest agency is Rapallo, what is best way to get there? thanks for any suggestions!
There is a train from La Spezia to Rapallo which makes a stop at Sestri Levante. I don't think you can catch a train to Sestri Levante from Manarola but I vaguely remember you can do that from Monterosso.
Hertz in La Spezia [15 minutes by train] and a Hertz rental; then about a 4 hour drive. Read this about driving in Italy and restrictions involved [ZTLs]; never leave anything of value in a vehicle when parked:

Train time to a town in the area, like Asti, will take over 5 hours.
The drive from Manarola to Alba takes 3 and half hours plus stops, if you drive through the mountains on the old A7.

There are trains from Monterosso to Asti taking a little less than 3 hours, fastest trains to Alba take less than 5 hours with two changes. Would it be possible the spend the last night in Monterosso? If not, Hertz has an office in Chiavari and Avis in Lavagna, but the most obvious place in the right direction to Alba is Genoa. Let Trenitalia take care of the first part of the journey and avoid backtracking to La Spezia: two birds killed with one train.
You will need take the train to Monterosso or Levanto. Avis, Budget and
Europcar are there.
??? I don't believe there are any rental offices in Monterosso.
