I feel your pain. I did mine a couple of weeks ago and it was crazy time-consuming. I'm helping my sister with it this weekend because she'll need assistance and we want to get it done. It seemed to be just going to Italy with the flight/train number, seat numbers and first accomodation. It hasn't kicked back yet!

CT, did you fill it out on your phone? How long did it take you? Also, I think we have to upload our passport, or do we just fill out info on it? I’m dreading this.

Also, what do you mean by flight/train number? I thought it was either/or.

Thank you Tdw. That was exactly our scenario. We are flying out of Indiana to JFL.

We are going JFK to Milan. Do you recall if you had to “upload” documents such as the CDC card or passport? I imagine I can just get this done anytime even if our trip is still 3 months away.

I did it on my computer and then installed the app on my phone. It drives me mad to do a lot of typing on my phone. I use multiple screens for work and could pull up info on one screen while typing on form. There is a lot of stuff to enter from flight numbers to seat numbers to the address of the first accomodation. I was sent an email confirmation.

We have a few months before we go and I'll keep tracking it.

I’m doing the right one. I was mistaken about uploading, I thought I had read that somewhere. I went through the instructions and it just asks for the passport info.

Ok, I filled it out. I was surprised that the Passport # was optional.

Question: since I’m filling this outside of 14 days like it says, will it be invalid and will I have to do it again? I was a little surprised it was accepted and it does show the date filled out.

CT, I’m having problems finding the app. I go to the Apple store and type in eudlf, and noting pops up. How did you access that?

I completed my DPLF but it appears that I can't simply add my wife to the same form, but rather she has to establish an entirely new form. Is that correct?

I completed our forms but haven't submitted them yet. We arrive in Italy May 9 from a Transatlantic cruise. When should I submit the forms? Is it too early to submit now?
