I believe the ticket “office” went up last week, before the May 1 holiday. I saw a picture in the Italian media of a booth across the Piazza from the entrance. I would say go early. Most tourists seem to turn out about 10 am.

As of a few days ago they had not started charging for entry. I'm not sure when this will go into effect.

I doubt they would charge for attending religious services. But if you do, you'd be expected to attend the services and not walk around and then leave. The same concept applies to the many churches in Europe that charge entry fees.

I was there today, May 15, and no admission was being charged. I looked around the piazza for a ticket booth but didn't see one.
