I guess to me it depends on where you are arriving from and what time your flight is the next AM.
Driving in from Florence. Arrive 3:00pm. Flight is at 11:40 in the morning.
Well, the train takes about 45 minutes, so there ought to be enough time to have dinner
in downtown Milan. But you might consider taking the train to Cadorna as opposed to
Centrale. A little closer to the Duomo, could do a little sightseeing as well, and probably
more options for dining.
Unless you're exhausted by the journey, it could indeed be a good idea to take a train to the city for dinner and a walk around.
I'd suggest looking at taking a train from Malpensa station (literally 20 yards from the Sheraton) to Milano Nord Cadorna. The journey time is 39 minutes (compared to 56 minutes to Milano Centrale). There are two trains per hour to both these destinations. The area around Milano Nord Cadorna is very pleasant, with a lot to see. It's on the Metro, and you can walk from there to the Duomo, La Scala or the Galleria Vittorio Emanuele in less than 20 minutes. Restaurants and bars abound.
In my opinion, the area around Milano Centrale is much less interesting. (One of the best things to see is the station structure itself!)
If you're tired, but still want to get away from the sad Sheraton with its overpriced and disappointing restaurant, you could do worse than taking a train to the small town of Saronno (where the well known amaretto disaronno liqueur originates, as do the also well known Amaretti di Saronno macaroons). Jouney time is only 18 minutes from the airport, with 6 trains departing every hour. Saronno has a lovely 'centro storico', which you could 'do' in an hour or two, before enjoying an aperitivo in one of its many bars, then having a slap-up meal in one of its numerous restaurants.
The Duomo, Galleria, and square between them are worth seeing. Best dinner I had in Milan was a place called Osteria Conchetta. No regrets.
I did this 2 years ago. Got the last slot to see DaVinci's Last Supper. Walked around, had dinner. I didnt love Milan itself, but it was a wonderful night and 100% worth it. Even though its tempting to relax before flying home, I try to always make my last evening count and not waste the evening!
