Putting Volterra with the car before Positano and Rome (without it) would have been easier, but I assume you have your reasons!

What Christine said. Car in Rome and Positano...never for me. As I look at your itinerary Volterra and Spello are the only two where a car is needed. You could streamline your itinerary by flying into Florence and out of Rome. Positano is an outlier. You've got plenty of time to plan.

I agree with Christine and Brad, you do not need a car in Rome or Positano. Unless you are traveling in the Chianti region, you don't need a car in Siena. Parking is a big challenge in Siena, especially if you are there during market day.

It looks like you are flying in and out of Florence, so unless you can change your plans to fly into Florence and out Rome, I might suggest that you go to Positano after Rome, take trains and pick up a car at the Rome Termini train station. It will be a bit hectic driving in the city, but that is the most convenient way for you to align transport.

Note that you are traveling during two big Italian Holidays. The 25th of April is Liberation Day and 1 May is Labor Day. We traveled those days last year and train transport was not a problem. However, we found our hotel rates were a bit higher and not all the restaurants and sights were open. Check where you are on those days to ensure you know what is open and what festivities are going on.
