Wondering what is more efficient: a direct bus from MXP to Genova or a train with a change in Milan. Also, Rome2rio says the bus service is Flixbus. Rode in it once several years ago without any issue, but asking for more current opinions. Travel day is a Monday in June during daytime hours.

It depends. There are only 3 bus runs during the day, at 9am, 12:45pm, 6:45pm, probably with limited seats (I think their buses carry only about 50 passengers).
Trains are more frequent (like about at least 2 every hour) and hold at least 400 passengers each. The overall journey times are similar and the price is also comparable, actually probably slightly cheaper by train since many trains to Genoa are Regionali or at most Intercity (I think there are very few FrecciaRossa as the MI-GE line is not high speed).
