To Florence, anything from 35 to 60 minutes according to traffic; traffic may get quite bad in the morning near the Florence airport or, in summer, at weekends. You could consider taking a train - just factor the price and difficulties of parking in Florence, and probably the train would be more reliable and cheaper.
Also, remember that Montecatini has a big train station but also a smaller train stop (Montecatini centro) near the center and the hotel district.
I have stayed in Montecatini numerous times and take the train into Florence because of the ZTL Areas and heavy traffic and lack of parking.ZTL Zones cannot be driven in unless you have a resident sticker,heavy fines,train into Florence is about one hour and 12 euros round trip for two people. I have driven from Montecatini to Monterosso in the CT but not as a day trip I believe it is 3-4 hours.

Kellie, curious if you have taken your trip. We are heading to Montecatini next week and have a similar itinerary. Would you share what your final trip entailed? Did you drive or take the train to Cinque Terre? Any other excursions you suggest?
How late are train returns from Florence back to Montecatini? That is my only concern with the train. We plan to take the train one day, but thought we might drive another so we can stay a bit later in Florence. Also,
