We have a day and night on October 20, in Milan at the end of our trip before flying out. Would like to find a small group walking tour to make the most of the day. Has anyone experienced "With Locals"? Other recommended tours?

We did a City Wonders last month, the 3 hour walking tour with Last Supper visit. It was not bad; the best parts were the tour of the Duomo, and the Last Supper visit. I would rate both of those "very good." The tour was €70 each.

About 20, I'd say, maybe 18 or 19, including a few children.

What we really like and will tip the decision is a tour group that uses the wireless headsets. That has been fairly common for the past several years.
