Would anyone recommend the three hour tour which includes the duomo and The Last Supper? I think that I would like to do this on our own and purchase the tickets for The Last Supper separately. Is it difficult to get there from the duomo?

No it is not difficult. You take the Metro from Piazza Duomo to Cadorna Station and walk about 15 minutes to Santa Maria della Grazie. You can do a Google Maps walking map to help guide you.
