We’ll be flying to Boston from Milan next Thursday. How is passport control leaving Italy? Three hours enough time to get through security and passport control and get to gate? We only have carry ons.
Your post is a bit confusing. If you are asking about leaving Milan Malpensa to come back to the US, 3 hours should be enough time to get thru airport security and passport control.
Barring some unforeseen circumstance (like a strike, or water main leak or something like that),
3 hours should be plenty of time. Note that it does take 45-60 minutes from downtown Milan
to MXP on top of that, but with carry-ons only, you can probably fudge the overall time calculation
a bit. Leaving Schengen should be a bit easier than entering it.
We have had issues with trains to Malpensa being canceled at the last minute, so allow for that also.

The Trenitalia ticket office at Milano Centrale train station sells Milano Express train tickets with a special supplemental service added to your train ticket that allows you to “skip the line” for Passport Control. It gets you through an extremely short special line for about €15. When we arrived, we were the only travelers in the line and the officer reviewed our passports and we were on our way in less than two minutes. It’s worth every penny if leaving through Milan Malpensa
