Have an early morning flight out of Malpensa airport in Milan. Looking for a hotel with airport transfer. Any suggestions?

I stayed at the MOXY last year - literally at the airport. It’s a clean modern hotel and had great rates.

The Marriott chain has one of their hotels right across from the train stop at T2 you can walk to. In the morning, there is a shuttle bus to T1 you can hop on for free from T2, about a 10 minute ride.

We've stayed at the Sheraton in Terminal 1 multiple times. No shuttle or transfer needed. Very easy access to Malpensa Express, too.

I highly recommend the Cardano Hotel. It’s a clean. modern business hotel with airport shuttles and the most amazing, helpful staff.
When we arrived at the hotel the evening before our flight home, we learned that the flight had been cancelled. After several frantic hours on the phone (thank you Skype!) we were rebooked on a flight early the next morning out of Linate Airport. It was now midnight, but the front desk clerk was able to arrange a taxi to pick us up at 5 a.m. And when we straggled into the lobby at 4:50 am, he had fresh croissants waiting for us, and prepared two steaming cappuccinos.
We were so grateful that we had picked a business hotel with a 24-hour desk!

We stayed at Osteria della Pista. Quiet, inexpensive, clean, modern rooms in a historical building, with a decent hotel restaurant and free transport to and from Malpensa. Nice people. We'd stay there again any time and we don't even like hotels. Not right at the airport, but very close. Check it out. Be sure to find out how early the first transfer van departs.
